What Is Ayushman Bharat PM-JAY?
Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (AB PM-JAY) is India's flagship public health insurance scheme, run by the National Health Authority (NHA) under the Ministry of Health and Family Welfare. Launched on 23 September 2018 in Ranchi, Jharkhand, it gives eligible families cashless, paperless hospital treatment at empanelled public and private hospitals across the country.
The scheme is often called the world's largest government-funded health assurance programme, covering an estimated 55 crore beneficiaries from roughly 12 crore economically vulnerable households, identified mainly through the Socio-Economic and Caste Census (SECC) 2011 data.
How Much Does PM-JAY Cover?
Each eligible family gets health cover of up to ā¹5 lakh per year, shared across all members with no cap on family size, age or gender. There is no premium to pay. The cover includes:
- Pre-hospitalisation expenses for up to 3 days
- Post-hospitalisation and follow-up treatment for up to 15 days after discharge
- Around 1,900+ medical procedures, including cardiac care, cancer treatment and joint replacements
- Coverage for pre-existing conditions from day one of enrolment
Treatment is cashless and paperless at any of the roughly 30,000+ empanelled hospitals nationwide, so beneficiaries don't pay upfront or file reimbursement claims.
Who Is Eligible?
Rural Households
A household qualifies if it meets any one of several deprivation criteria under SECC 2011, such as living in a kuccha (mud/temporary) one-room house, having no adult male member aged 16ā59, or being headed by a woman with no adult male in that age range.
Urban Households
Eligibility is based on occupational categories identified as vulnerable under SECC 2011, such as rag-pickers, domestic workers, street vendors, and similar informal-sector occupations.
Senior Citizens (70+)
Since October 2024, every Indian citizen aged 70 and above is eligible for PM-JAY cover of up to ā¹5 lakh a year through the Ayushman Vay Vandana Card ā regardless of income or existing SECC status. This runs as a top-up: if a senior's family already has separate PM-JAY cover through SECC eligibility, the 70+ cover is available in addition to it.
Seniors already enrolled in CGHS, ECHS or Ayushman CAPF must choose between staying on their existing scheme or switching to PM-JAY ā the two cannot be combined. Anyone already covered by private insurance or ESI can use PM-JAY on top of that cover.
How to Check Eligibility and Apply
- Visit the official portal at pmjay.gov.in or download the Ayushman App
- Click "Am I Eligible," enter your mobile number and complete OTP verification
- Select your state and enter your name, ration card number, or RSBY URN to search the beneficiary database
- If eligible, complete Aadhaar-based e-KYC to generate your Ayushman Card
- Visit a nearby Common Service Centre (CSC), empanelled hospital help desk, or Ayushman Mitra kiosk if you need assistance completing the process offline
"Interest in expanding PM-JAY's senior citizen coverage reflects a broader shift toward universal health security for India's elderly population." ā National Health Authority, 2026 update
Using Your Ayushman Card at a Hospital
At an empanelled hospital, present your Ayushman Card along with Aadhaar for verification. The hospital's dedicated Ayushman Mitra desk handles biometric or OTP authentication and pre-authorisation directly with NHA ā the beneficiary does not pay for covered treatment or file paperwork afterward.
Common Mistakes to Avoid
- Assuming automatic enrolment: Being in an eligible SECC category doesn't generate a card automatically ā you must complete e-KYC to activate it.
- Skipping empanelled hospitals: Treatment at a non-empanelled hospital isn't covered under PM-JAY.
- Not verifying details before treatment: Mismatched Aadhaar details can delay authorisation during a medical emergency ā it helps to verify your card details in advance.
- Forgetting to check state participation: A few states and union territories, including Delhi and West Bengal, have not implemented PM-JAY as of 2026; residents there should check their own state health scheme instead.
What PM-JAY Does Not Cover
PM-JAY is built for hospitalisation, not routine outpatient care. It generally excludes:
- Outpatient consultations and medicines not tied to a covered hospital admission
- Cosmetic procedures and fertility treatments
- Organ transplant donor costs (recipient treatment may be covered under specific packages)
- Treatment for conditions requiring only diagnostic tests, with no hospital admission
For ongoing outpatient needs, PM-JAY works best alongside your state's public health centres (Ayushman Arogya Mandirs), which handle primary care, screening and chronic-disease management separately from the hospitalisation cover.
PM-JAY vs Private Health Insurance
PM-JAY is a strong safety net, but it has a fixed annual cap and a limited procedure list, decided centrally rather than customised to an individual's health profile. Families who can afford it often treat PM-JAY as a base layer and add a private policy for outpatient cover, higher sums insured, or treatments outside PM-JAY's package list. The two aren't mutually exclusive for most beneficiaries, so there's rarely a reason to choose only one.

What changed in 2026
Since October 2024, PM-JAY has covered all citizens aged 70 and above through the Ayushman Vay Vandana Card, regardless of income. By 2026 this expansion had pushed total Ayushman Cards issued past 27 crore, with the empanelled hospital network growing past 30,000 facilities nationwide.
Frequently asked questions
QIs Ayushman Bharat PM-JAY free?
Yes. Eligible beneficiaries pay no premium and no cost for covered treatment at empanelled hospitals. The scheme is fully funded by the central and state governments, with cashless, paperless treatment up to ā¹5 lakh per family per year.
QAre all senior citizens automatically covered under PM-JAY?
Since October 2024, everyone aged 70 and above qualifies for PM-JAY cover regardless of income, through the Ayushman Vay Vandana Card. However, you still need to complete e-KYC on pmjay.gov.in or the Ayushman App to activate the card and use the benefit.
QCan I use PM-JAY along with private health insurance?
Yes. If you already have private health insurance or ESI coverage, you can still use PM-JAY on top of it. However, those enrolled in CGHS, ECHS or Ayushman CAPF must choose one scheme, since these cannot be combined with PM-JAY.
QWhich hospitals accept the Ayushman Card?
Only hospitals empanelled under PM-JAY accept the card for cashless treatment ā over 30,000 public and private hospitals nationwide as of 2026. You can check the empanelled hospital list for your area on the official pmjay.gov.in portal or the Ayushman App.
QWhat documents do I need to apply for an Ayushman Card?
You typically need Aadhaar for e-KYC, along with your ration card number or family ID for SECC-based verification. Age 70+ applicants generally need only Aadhaar and proof of age, since eligibility no longer depends on income category.
